Telehealth & Medical Disclaimer

Last updated: January 16, 2026

1. No Medical Advice

HNC provides technology platforms and digital tools designed to support healthcare providers in delivering TeleHealth, remote patient monitoring, and related clinical services. However, the platform itself does not offer medical advice, diagnosis, treatment recommendations, or clinical interpretations. Any information displayed within the App or Services—such as readings, messages, reminders, or educational content—is intended solely to facilitate communication and data exchange between you and your licensed healthcare provider.

You should never rely on the App as a substitute for professional medical judgment. Only a qualified healthcare provider can evaluate your symptoms, interpret your health data, and determine the appropriate course of care. If you have questions about your health, medications, or treatment plan, you must consult your provider directly. The App is a supportive tool, not a clinical decision-making system.

2. Emergency Situations

The Services are not designed or intended for use in urgent, critical, or life‑threatening situations. The platform does not provide real-time monitoring for emergencies, nor does it alert emergency responders or guarantee immediate communication with your healthcare provider. Delays in data transmission, connectivity issues, or device malfunctions may occur, making the Services unsuitable for emergency care.

If you believe you are experiencing a medical emergency—such as chest pain, difficulty breathing, severe bleeding, sudden confusion, or any condition requiring immediate attention—you must call your local emergency number right away (such as 911 in the United States). Do not rely on the App, messaging features, or remote monitoring tools to obtain urgent medical assistance.

3. Provider Relationship

All medical decisions, diagnoses, treatment plans, prescriptions, and clinical interpretations are made solely by licensed healthcare providers. HNC does not employ or supervise healthcare professionals and does not participate in the clinical decision-making process. Our role is limited to providing the technology that enables providers to access patient information, communicate with patients, and manage care remotely.

Your relationship with your healthcare provider remains independent of HNC. Any questions about your care, treatment options, or medical concerns must be directed to your provider. HNC does not review, verify, or validate clinical information entered by providers or patients, and we do not intervene in the provider‑patient relationship.

4. Telehealth Limitations

While TeleHealth and remote monitoring can enhance access to care, they also have inherent limitations. Technology-based services may be affected by connectivity issues, device malfunctions, incomplete or delayed data transmission, or environmental factors that impact the accuracy of readings. These limitations may affect your provider’s ability to fully assess your condition.

TeleHealth encounters may not always provide the same level of detail as in‑person examinations. Certain symptoms, conditions, or concerns may require physical evaluation, diagnostic testing, or direct observation that cannot be performed remotely. Your provider may determine that an in‑person visit is necessary to ensure safe and effective care.

5. No Guaranteed Outcomes

HNC strives to provide reliable, secure, and high‑quality technology solutions to support healthcare delivery. However, we do not guarantee that the use of our Services will result in improved health outcomes, prevention of medical conditions, or successful treatment results. Health outcomes depend on numerous factors, including clinical judgment, patient adherence, underlying health conditions, and circumstances beyond the control of HNC or the technology platform.

The Services are intended to supplement—not replace—professional medical care. While remote monitoring and digital communication may enhance care coordination, they cannot ensure specific results or eliminate all risks associated with medical treatment.
